Add Date of Birth as option in the Add New Individual pop up

Especially in the Health Insurance world Date of Birth is extremely important. Because it does not appear in the pop up of Add New individual, it often gets forgotten to be added. It does not have to be a required field, just should show up as one of the fill option in Add New Individual. Thanks! Melanie
